1.Register for payment via VISA or MASTER CARD Credit or Debit card from all banks

You can register to make auto payment by credit or debit card with the following steps:

1. The credit card owner sign and fill in the consent form, the assured or the person who connects to the assured such only father, mother, husband or wife, and son or daughter. The person must not accommodate or related to the money laundering or financial support for terrorism including international monetary boycott
   Download the payment via credit or debit card consent form (for Life Insurance)
2. Support Documents
   1) Photocopy of credit card (both side) and signed to approved the legit of the document by the credit card owner
   2) Photocopy of your national ID card and states the will of the activity for this document and signed to approved
3. Complete the form by fill in and your signature.
4. Send to the Company by select one of the following

Term and condition acceptance

• Credit card that can be applied for premium payment: VISA / MATER CARD/ JCB from all bank issuer/ financial institute
• To complete the premium payment will occur once the Company can receive the payment from the Bank/ Financial institute in full amount
• For automatic payment condition, if the Company cannot collect/ deduct the payment from credit card on due date. The payment transaction will be considered uncompleted and the Company will attempt to process collecting on the following day, or until the payment status for the installment is completed by automatic loan or insurance out of date.

2. Change Credit or Debit card service

 If you want to change credit or debit card for auto payment, please contact and inform the change before the due date of payment in advance for minimum 5 working days to prevent the auto premiums payment with the current card

If the information to change credit or debit card for auto payment, less than 5 working days. The system will deduct the amount from the current card in the system, then the amount will be charge automatically to deduct the premiums amount with the new account from next payment due onward.

Registration and support documents to change the credit / Debit card for auto premiums payment;

1.  The assured fill in the form to change of alter the details in the Insurance
   Download the form to change or alter the details of the Insurance (Non-Finance)
2. Document supports;
   2.1 the photocopy of credit/debit card (both side) and signed to approved the legit of the document by the credit card owner
   2.2 Photocopy of your national ID card and states the will of the activity for this document and signed to approved
   2.3 In case the assured is not over 15, please provide birth certificate or House registration together with the photocopy of national ID card of the parent, the payer.
   2.4 In case of changing Name – Surname, photocopy the certificate of changing name -surname of the assured
3. Complete the form by fill in and your signature.
