Consent Form: For Collection, Use, and Disclosure of Personal Information
I hereby consent to allow Generali Life Assurance (Thailand) PLC. and Generali Insurance (Thailand) PLC., hereinafter called “Companies” or “we”, as the Data Controllers, to collect, use, and disclose my personal information in accordance with the following provisions:
1. Personal information which will be collected, used, and disclosed by Companies:
Your personal information received by Companies, specifically age, address, gender, occupation, nationality, phone number, ID number, email address, Bank account, credit card number, Line ID, salary, location, Facebook information/account, Line account, website cookies, etc., may be used in accordance with the purposes specified in this form.
Companies will provide for strict security measures to protect your personal information and prevent any person from accessing and using your personal information in any way not conforming to this form. Companies will also ensure that your personal information is maintained in complete, accurate, and updated manner to the extent that we can.
2. Purposes
The purposes for the collection, use, and disclosure of your personal information are to offer to you for sale any type or form of insurance/life insurance products, send any news and advertisement, analyze your personal information for improving our sale.
3. Duration of collection, use, and disclosure
3.1 In case you have no insurance policy with us, your personal information will be stored and used for the purposes in clause 2 for 10 years from the date on which this consent is given; or
3.2 In case you have insurance policy with us, your personal information will be stored and used for the purposes in clause 2 until you are 71 years old.
4. Lists of persons or organizations to which your personal information may be disclosed for the purposes outlined above, and who may assist us with the purposes outlined above
1. Companies and our group companies inside and outside of Thailand;
2. Our agents, brokers, and business partners inside and outside of Thailand;
3. Our service providers dealing with our IT, marketing, advertising, PR, data analysis, and research and product development inside and outside of Thailand, including but not limited to, Google and Facebook and their affiliates, etc.
5. Your Rights as Data Owner
You have the following rights:
(1) right to accurately update your personal information;
(2) right to withdraw your consent given herein;
(3) right to object to collection, use, and disclosure of your personal information;
(4) right to request for suspension of collection, use, and disclosure of your personal information;
(5) right to request to delete or destroy your personal information;
(6) right to access your personal information;
(7) right to request for a certified copy of your personal information;
(8) right to verify the method of gaining your personal information; and
(9) right to complain against us for our breach under this form or the Personal Data Protection Act B.E. 2562 and relevant regulations.
